Many use topical estrogens and androgens to help control their own case of male pattern baldness. They found that the use of the estrogens alone tends to lead to scalp fragility and tenderness, whereas the androgens alone lead to scalp acne. To avoid these side effects, people use a 1 to 15 mixture of estradiol dipropionate to testosterone cypionate in either reagent grade DMSO (preferred) or a 50-50 mixture of acetone and ethyl alcohol as a solvent—a combination that yields somewhat better results than either alone. Each drop of solvent contains 2 micrograms of estradiol dipropionate
and 30 micrograms of testosterone cypionate. Each treatment uses about 30 to 60 drops; 10 to 20 on top of the head, and 10 to 20 more at each front side corner, the areas where their hair was receding.
The solution is rubbed into the scalp and allowed to dry. About one treatment per week of about 120 micrograms of estrogen and 1.8 milligrams of androgen has halted the progress of many peoples’ balding. These doses of sex hormones are far too low to have any systemic effect, but they seem to have worked when applied directly to the scalp.
At this point a brief technical note on the drug delivery system is in order. Ideally, the androgens and estrogens should be delivered only to the hair follicles in the balding area, without appreciable whole body dosage. The proper solvent-hormone system can come close enough to this goal.
Early experiments reported in the literature used hormones such as testosterone propionate, which diffuse quite rapidly. When applied to the scalp in an absorption base, the hormone rapidly moves out into the remainder of the body, resulting in both an insufficient amount in the scalp and an undesirable excess in the body as a whole.
The cypionate and dipropionate esters should be used because they have a much lower mobility in the body; they are sometimes used as depot injections or implanted pellets which slowly leak hormones over a period of weeks or months. Indeed, the dipropionate esters are so immobile that they are usually unsuitable for depot injections. These esters dissolve well in fats but are very insoluble in water.
The solvent used to deposit these hormone esters in the scalp must be low in toxicity, high in fat solvent power, able to rapidly penetrate the skin, and miscible (mutually soluble) with water. DMSO meets these requirements very well. The testosterone cypionate and estrodial dipropionate dissolve
slowly in dry DMSO but precipitate out of solution when a small amount of water is added. This is exactly what happens when the solution is applied to your scalp. The DMSO carries the hormone esters through the dry waxy oily dead outer layer of skin. When it reaches the upper layer of living cells,
however, their high water content causes the prompt precipitation of the hormone esters right where you want them. The DMSO soon spreads throughout your body, but most of the very hydrophobic hormone esters are left behind in the scalp where they belong.
Both the quantity and frequency of application of this treatment must be individualized. If you use too little too infrequently, your hair loss will be unaffected. If you use too much too often, you will have acne on your scalp. Here is how you adjust your dose. Comb your hair regularly and save all
hair fallout, using a separate envelope each day. You will soon discover by estimation (or preferably by actual hair-root count) your typical daily hair loss rate. The proper topical dose of hormone solutions will cause a reduction in hair fallout rate of at least a factor of four (and usually more like a factor of ten to twenty) within two days, with a noticeable effect within twelve hours, and the treatment will remain effective for about a week. When the hair loss rate rises, another application is used.
Do not confuse the approach described above with the early testosterone propionate experiments which, while interesting, were of marginal effectiveness and had unacceptable systemic side effects.
